Step 1: Download PPP Stimulus Loan Application*

If you are an Independent Contractor, please skip step one and two and contact a lender.

Please check your Downloads folder after download is complete.

*Pending SBA Approval, some additional documents may be required.

Step 2: Complete and save the application. Have payroll and business entity documentation ready to send when an EB Lender notifies you.

Within 24 hours, after completing step 3, you should receive an email containing a secure upload link to send us your completed PPP application.

Step 3: Provide EB Lender with your contact and business information.

Due to the high volume of applicants, response time could be up to 24 hours.

Loan application (SBA Form 5), completed and signed (this is electronic/online in the portal) • Form 1368, Additional Filing Requirements EIDL
Tax Information Authorization (IRS Form 4506-T), completed and signed by each applicant, each principal owning 20 percent or more of the applicant business, each general partner or managing member; and, for any owner who has more than 50 percent ownership in an affiliate business. Affiliates include, but are not limited to, business parents, subsidiaries, and/or other businesses with common ownership or management
Complete copies, including all schedules, of the most recently filed Federal income tax returns for the applicant business, an explanation if not available
Personal Financial Statement (SBA Form 413) completed, signed, and dated by the applicant, each principal owning 20 percent or more of the applicant business, and each general partner or managing member
Schedule of Liabilities listing all fixed debts (SBA Form 2202 may be used)

According to SBA.GOV, The estimated time for completing this entire application is two hours and ten minutes, although you may not need to complete all parts.
